• For minor applicants, please bring [1] NSO/PSA birth certificate; or [2] Report of Birth (ROB) filed at the Philippine Embassy in Wellington of those born in New Zealand. Also bring copies of parents' or at least, mother's passport (one per child). Deadline of submission: 20 October 2022.
• For women applicants who wish for their married name to be reflected on their new passport, please bring [1] NSO/PSA marriage certificate; or [2] Report of Marriage (ROM) if filed at the Philippine Embassy in Wellington. Deadline of submission: 20 October 2022.
